The Process
1. Telephone Consultation (or in person):
Our senior staff members will initially take your input on the care specifications and details required by you such as the location, frequency, type of care and hours needed and arrange for a personal assessment.
2. Physical Assessment:
Having learned about your needs, we will then arrange for a senior member to visit you in your home to conduct a thorough assessment of the patient and the environment to determine the conditions and the environment under which care will be provided.
3. Care Plan:
Having run an assessment, we will present to you an individually-tailored care plan consisting of the exact functions and tasks required with details and schedules of carrying them out. Such plan is designed to reflect the care requirements set out by service users. This is then followed by the commencement of care provision
